Exactly, and when he finally did something really horrible like the Batman Beyond Movie, it hit hard. Now people are just numb to it all.

Armchair psychiatrists did the worst damage to the Joker. Once it was decided he was "insane", all bets were off. Hannibal Lector in clown make-up. And it WAS make-up, thanks to Frank Miller, despite half a century of prior evidence.

So, a bunch of people with no training in mental illness whatsoever began writing their versions of an "insane" Joker, and the character was lost.

I only just learned that Arkham Asylum wasn't created until the 70's/80's, and only truly evolved into its present state during the late 80's. That's also a part of the problem - now pretty much every one of Batman's rouges' gallery is considered to be insane - and that provides fodder for making them even more heinous.

In the distant past, the question of "Why doesn't Batman just kill the Joker?" could rightfully be easily dismissed. But after years of writers making it more personal, with Joker killing and torturing countless others - including several people close to Batman - it starts to become a logical question. Of course Batman can't kill Joker regardless because that means no more Joker stories - so it ends up adding to the bad feedback loop of Batman also being deranged for allowing this to continue.

In the end, this mess is all on the editors for allowing the writers to make it so bad.

John Byrne: Armchair psychiatrists did the worst damage to the Joker.

…..

Armchair psychiatrists also did the worst damage to Batman. There have been way too many writers who have attempted to psychoanalyze Batman over the past 30 years, depicting him as a brooding monomaniac control freak, or arguing that he's just as crazy as his rogues gallery.
