The most fake aspect is the signatures. It's obviously one that didn't exist on the original art and it doesn't match how the artists signed at the time.Austin definitely signed with non-caps letters (not sure if he ever signed otherwise). If anyone has evidence to the contrary, I would like to see it.

Malcolm Gladwell’s book BLINK comes to mind.
My first reaction was “FAKE” and though I do wonder about some
aspects, there are too many details that feel off for me to be convinced
it’s real without overwhelming evidence.

I think barring someone who was at the Marvel offices then recognizing it should be considered as very dubious. It's not just any issue but up there with Hulk #181 as a 'key', and while that comic's other cover has been known of, this one does not seem to have been at all known to exist before now.
DC's Detective Comics #500 had a list of contributors on the front cover, also Spirit magazine #30... neither being Marvel books.
I'd vote fake at this point too.

At first glance, it looks like the cover for an RPG manual. I wonder why anyone would make a fuss, anyway, since it’s not a published piece, and not original artwork.

My initial reaction was 'fake'. And I haven't changed my mind in the last few days.
The 'novel-length' claim seems odd.

A term that began when comics typically had three or four stories. When a tale filled the whole issue, it was often called “novel length”. Tho as I recall it was more typically DC that used it. The cover credits are unusual for the vintage, too. Altho titles and stories were sometimes credited as “award winning” or some variant, crediting the talent on the covers was discouraged. The books, it seemed, sprang fully grown from Zeus’ head.

The Marvel UK reprint series tended to have several series
reprinted each issue - so stories would be split across
several issues - typically between 3 or 4 issues.
When they started to include whole issues, they sometimes
referred to 'novel length' stories being included.
